Terms and conditions
Life
1. This Offer is open to all persons who comply with these Terms and Conditions. For the purposes of these Terms and Conditions:
(a) “Additional Condition” means the Eligible Participant must have earned 48 Vitality Points per month for the two consecutive months following the Inception Date (the date the cover begins). For the avoidance of doubt, where a VitalityLife Plan has two Eligible Participants (i.e. is a “joint life” VitalityLife Plan) both Eligible Participants must meet the Additional Condition separately.
(b) “Cashback” means the value of one month’s Premium of the Eligible Participant’s VitalityLife Plan, subject to the restrictions in clause 4.
(c) “Eligible Participant” means an individual who is covered under an Optimised VitalityLife Plan which:
(i) is applied for during the Offer Opt-in Period;
(ii) has an Inception Date between 1st July 2021 and 30th November 2021 (inclusive);
(iii) has all relevant Premiums paid up to date;
(iv) is the first person covered on the VitalityLife Plan. On a Joint Life Plan, this is the first and second person covered as defined in the plan provisions;
(v) has provided a valid email address to Vitality; and
(vi) is in force or active.
For the avoidance of doubt, this Offer is applied automatically to all Eligible Participants who comply with these Terms and Conditions.
(d) “Inception Date” means the date on which the insurance cover (excluding Immediate Cover and Mortgage Free Cover) commences for the lives insured under the VitalityLife Plan.
(e) “Initial Conditions” means the following conditions:
(i)the Eligible Participant must complete the “Online Health Review” for the relevant VitalityLife Plan within three months of the Inception Date; and
(ii)the Eligible Participant must have earned at least three Vitality Points under the relevant VitalityLife Plan within three months of the Inception Date.
For the avoidance of doubt, where a VitalityLife Plan has two Eligible Participants (i.e. is a “joint life” VitalityLife Plan) both Eligible Participants must meet the Initial Conditions separately.
Please note that Initial Condition (i) will be considered met where the Eligible Participant is an existing Vitality customer, and has completed an Online Health Review within the 15 months prior to the Inception Date.
(f) “Offer Opt-in Period” means 1st July 2021 to midnight 30th September 2021.
(g) “Online Health Review” means an online Vitality age assessment that can be accessed and completed on Member Zone.
(h) “Optimised” refers to a VitalityLife Plan which includes Optimiser
(i) "Premium(s)" means the total plan premium paid in order to maintain coverage under their VitalityLife Plan.
(j) "Premium-Payer" refers to the person responsible for paying Premiums on the VitalityLife Plan.
(k) “Vitality Point(s)” means points awarded to Eligible Participants under their VitalityLife Plan for any points-earning physical activity (as specified by Vitality).
(i) “Vitality” means Vitality Corporate Services Limited (registered number 05933141) of 3 More London Riverside, London SE1 2AQ.
(m) “VitalityLife Plan” means a pure protection insurance contract, administered by Vitality trading as VitalityLife. For the purposes of these Terms and Conditions, the VitalityLife Plan must be genuinely new (so cancelled/lapsed and re-keyed VitalityLife Plans will be excluded).
When Cashback can be earned
2. Subject to clause 4, the Premium-Payer will be entitled to receive the Cashback under these Terms and Conditions where the Eligible Participant meet the Initial Conditions.
3. Subject to clause 4, where the Eligible Participant meets the Initial Conditions and the Additional Condition, the Premium-Payer will be entitled to receive a total of double the amount of Cashback (i.e. the value of two month’s Premiums).
Restrictions on the Cashback
4. A Premium-Payer who is eligible to receive the Cashback under clauses 2 or 3 will only be entitled to receive a maximum of £500 in Cashback, regardless of the value of their Premiums or the number of VitalityLife Plans they own or are covered under.
5. Where an Eligible Participant is covered under multiple VitalityLife Plans purchased during the Offer Opt-in period, they will be able to utilise this Offer on all VitalityLife Plans subject to clause 4.
6. The Cashback will be paid across to the Premium-Payer within 5 months of the Inception Date, provided that the Eligible Participant is still in compliance with these Terms and Conditions. The amount of the Cashback payable will be calculated based on the lowest Premium per VitalityLife Plan during this period.
7. If an Eligible Participant’s VitalityLife Plan lapses or is cancelled at any point during the 12 month period following the Inception Date, the Premium-Payer must repay any Cashback they received to Vitality.
8. To be eligible for the Incentive, the Eligible Participant must provide Vitality with their email address. Vitality will not be liable for any lost electronic codes in the event that an Eligible Participant has changed their email contact details, and failed to notify Vitality of this change.
9. When taking part in the Incentive, the Eligible participant acknowledges and consents that their name and email address (“Personal Data”) will be made available to Vitality for administrative purposes. All Personal Data of the Eligible Participant will be held and used for the purposes of this Incentive by Vitality in accordance with Vitality’s privacy policies (which can be found onhttps://www.vitality.co.uk/privacy/ or if not, requested from Vitality) and data protection legislation.
10. Vitality reasonably suspects any Eligible Participant and/or Premium-Payer to have committed or attempted to commit an act of fraud or dishonesty, the Premium-Payer will repay the Cashback to Vitality upon request if this has already been received by the Eligible Participant.
11. Any decisions made by Vitality over eligibility or otherwise in relation to the Cashback are final, will not be open to dispute and no correspondence will be entered into.
12. Vitality is not liable to any person in any way in relation to this incentive, except for any liability that cannot be excluded by law.
13. Vitality reserves the right to vary, extend or withdraw this incentive at any time, including in the event of fraudulent behaviour by any Eligible Participant and/or Premium-Payer.
14. Vitality reserves the right to amend these Terms and Conditions on the provision of written notice or via Vitality’s web site.
15. These Terms and Conditions shall be governed by English law and the parties agree to submit to the exclusive jurisdiction of English and Welsh courts and tribunals with regard to any dispute or claim arising under these Terms and Conditions.

Personal healthcare
This offer is open, and applied automatically, to all Participating Members of an Eligible Plan who comply
with these Terms and Conditions. For the purposes of these Terms and Conditions:
a) “Cashback” means the amount payable to the Principal Member based on Premiums paid by the Principal Member and calculated in accordance with clause 3;
b) “Cashback Unit” means the value of one month’s Premium, calculated as an average of the Premiums due for the first three months of the Eligible Plan.
c) “Eligible Plan” means a VitalityHealth Personal Healthcare Plan that has:
i. a Plan Start Date between 1 July 2021 and 30 September 2021 inclusive; and
ii. all Premiums paid up-to-date at the Payment Date;
For the avoidance of doubt, VitalityHealth Personal Healthcare Plans that include any members who were already insured at any time between 15 May 2021 and 30 June 2021 (inclusive) will not be eligible for the Offer:.
d) “Engagement Period” means the calendar month in which the Plan Start Date commenced, until the end of the following two calendar months;
e) “First Qualifying Condition” means that the Participating Member has – subject to clause 7 –during the Engagement Period:
i. completed all sections of the online Health Review (Vitality Age) assessment, and earned Vitality Points for that completion; and
Ii. earned at least one Vitality Point for physical activity;
f) “Participating Member” means any person that is covered under an Eligible Plan that:
i. is aged 18 or over when their cover under the Eligible Plan began;
ii. joined the Eligible Plan during the Engagement Period; and
iii. remains covered on the Eligible Plan on the last day of the Engagement Period;
g) “Payment Date” means the date on which the Cashback is paid to the Principal Member, this being no later than the end of the fifth calendar month following the Plan Start Date:
h) “Plan Start Date” means the date on which insurance cover first began on the Eligible Plan;
i) “Premium” means the monthly amount paid by the Principal Member to VitalityHealth in order to maintain coverage for a VitalityHealth Personal Healthcare Plan;
j) “Principal Member” means the person responsible for paying the Premiums on the Eligible Plan;
k) “Second Qualifying Condition” means that the Participating Member has – subject to clause 7 – during the
Engagement Period:
i. completed all sections of the online Health Review (Vitality Age) assessment, and earned Vitality points for that completion; and
ii. earned at least 48 Vitality Points for physical activity in each of the two calendar months following the month of the Plan Start Date;
l) “Vitality” means Vitality Corporate Services Limited (registered number 05933141) of 3 More London Riverside, London SE1 2AQ;
m) “VitalityHealth Personal Healthcare Plan” means a general insurance contract for the provision of private medical healthcare, administered by Vitality trading as VitalityHealth; and
n) “Vitality Points” means points awarded to a Participating Member under their VitalityHealth
Personal Healthcare Plan for healthy living activities (as specified by Vitality).
2. Where all Participating Members meet the following conditions the Principal Member will be entitled to receive the Cashback as follows:
i. if all Participating Members meet the First Qualifying Condition the Principal Member will
be entitled to receive one Cashback Unit; or
ii. if all Participating Members meet the Second Qualifying Condition the Principal Member will be entitled to receive two Cashback Units.
3. The Cashback amount will be calculated at the start of the fifth month following the Plan Start Date. This allows a period of one full calendar month after the end of the Engagement Period for any Vitality Points for physical activity earned during the Engagement Period to be submitted to Vitality, and for any membership changes that might affect the Premium.
4. The Cashback Unit(s) will be paid on the Payment Date, provided that the Principal Member is still in compliance with these Terms and Conditions.
5. If an Eligible Plan lapses or is cancelled at any time during the 12 month period following the Plan Start Date, the Principal Member must immediately repay any Cashback they received to Vitality.
6. If Vitality reasonably suspects any Participating Member to have committed or attempted to commit an act of fraud or dishonesty, the Principal Member will repay to Vitality upon request the Cashback if this has already been received by the Principal Member.
7. If a Participating Member already has another VitalityHealth, VitalityLife or VitalityInvest plan, and has completed their online Health Review (Vitality Age) assessment in the 15 months prior to the end of the Engagement Period, they will not need to complete the Health Review (Vitality Age) assessment during the Engagement Period.
8. Any decisions made by Vitality over eligibility or otherwise in relation to the Cashback are final, will not be open to dispute and no correspondence will be entered into.

m) “VitalityHealth Personal Healthcare Plan” means a general insurance contract for the provision of private medical healthcare, administered by Vitality trading as VitalityHealth; and
n) “Vitality Points” means points awarded to a Participating Member under their VitalityHealth
Personal Healthcare Plan for healthy living activities (as specified by Vitality).
