Confirm a Vitality activity

Find out how to notify us if you’ve completed an activity that we can’t track, so that we can reward you with Vitality points

There are some activities that we can’t track, like a visit to the dentist or your GP. If you’ve had a check-up or test that’s listed in the Vitality points guide and meets the eligibility criteria, send us evidence of the activity and we’ll update your Vitality points statement.

Recently visited the dentist? 

Use our online form to confirm a dental check-up.

For all other activities follow the steps below. 

We’ll award Vitality points for the following activities if evidence is sent to us within six months of the activity:

  • Completed vaccinations
  • Mammogram and PAP smears
  • Prostate screening
  • Health screening
  • Fitness assessments 
  • Eye test – result needs to come from an optometrist or ophthalmologist, not an optician
  • Faecal Occult Blood test / Faecal Immunochemical test / Abdominal aortic ultrasound
  • Abdominal aortic ultrasound

Please email evidence of the activity to vitalityresults@vitality.co.uk along with the following details:

  • Name of the member who had the appointment
  • Vitality membership number
  • Details of the appointment e.g. mammogram, prostate screen
  • Results e.g. BMI measurement, blood pressure reading
  • Date of the appointment
  • The name and address of the centre where the activity took place

Please note:

  • Evidence could be a copy of your results, a copy of your receipt with the doctors name and address and details of the check-up or test.
  • You need to send us evidence within six months of the activity so you don’t lose your points.
  • Most of our screening and fitness assessment partners will send us your results so you don’t need to. Please check at your appointment.

VitalityLife members - confirming your HBA1C results

If you're diabetic and have recently attended an appointment with your GP to measure your HbA1c, please email a copy of your HbA1c results provided by your GP to lifecustomerservices@vitality.co.uk along with the following details:

  • Name of the member who had the appointment
  • Vitality membership number e.g. 12234540384
  • Plan number e.g. 90876543
  • The result of the HbA1c screening
  • Date of the appointment
  • The name and address of the centre where the appointment took place

We’ll use these results to set your Wellness Status at your next plan anniversary.
